Qualifications

• Minimum 2 years of experience in behavioral health, social services or human services field.
• Minimum 2 years of case management experience.
• Willing and able to travel up to 75% within Lake County IL and surrounding areas.
• Reliable transportation; mileage reimbursed per policy.

Responsibilities

• Evaluate referred members and determine needs/eligibility and appropriate care plan.
• Coordinate and monitor assigned care plan activities and progress.
• Collaborate with case managers, Medical Directors and other health programs to optimize outcomes.
• Ensure compliance with regulatory and accreditation guidelines.
