Job summary

Humana Inc. is seeking a Case Manager to support Wisconsin Family Care members in the Ashland/Superior area. You will partner with a Field Care Nurse to create and update a comprehensive Member Care Plan, conduct in-home assessments, and coordinate social and non-medical services while ensuring cost-effective care.

The role involves up to 40% travel, requires a valid driver's license, and includes benefits described by Humana. Typical schedule is Monday through Friday, with a 40-hour work week.

Qualifications

• Bachelor's degree in human services or related field is required with at least 1 year of experience serving frail elders or adults with disabilities.
• Alternatively, a Bachelor's degree in another field with at least 3 years of such experience is acceptable.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address members' needs in the least restrictive environment.
• Conduct face-to-face social assessments upon enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical support services including social integration, housing, employment, and community resources.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide member education.
• Maintain accurate documentation including case notes and MCP updates.
• Use your skills to make an impact.
