Bay Area Community Services (BACS) is seeking a Clinical Care Coordinator in Solano County, based in Fairfield, to provide high-quality therapeutic interventions and services supporting wellness and recovery. Duties include case management, crisis intervention, housing navigation, and family support.

Formación

• Master's degree in Social Work, Marriage and Family Therapy, or Clinical Counseling.
• Active California licensure or registration (ASW/LCSW or MFT/LMFT) with BBS.
• Minimum 2 years of social service experience.
• Bilingual in a threshold language preferred.
• Valid California driver’s license with personal vehicle and insurance.
• Extensive local travel required; vehicle reimbursement provided.

Responsabilidades

• Develop and deliver counseling, case management, and rehabilitation services for assigned individuals.
• Apply evidence-based practice to achieve high-quality results.
• Assess risk and create person-centered service plans.
• Coordinate care and engage clients; monitor progress and engagement.
• Document data and charting per regulations; meet monthly service-unit targets.
• Participate in supervision, meetings, travel, and on-call coverage.
