Job summary

Bay Area Community Services is seeking a Care Coordinator/Case Manager to deliver evidence-based therapeutic interventions and case management for chronically homeless individuals in San Mateo County. Travel throughout the county and transport clients in your own vehicle are required as part of service delivery.

The role emphasizes housing navigation, crisis intervention, and family support within the ACT/FSP framework, with a schedule Sun-Thurs or Tue-Sat.

Qualifications

• High School Diploma required; 4 years of paid work experience in mental health/social services.
• One year may be in customer service.
• Degree is preferred.

Responsibilities

• Develop and deliver a full complement of services including individual, family, group counseling, and case management for individuals on caseload.
• Utilize Evidence Based Practice to assure high quality results.
• Assess risk status and intervene appropriately.
• Create service plans focused on recovery and client goals.
• Engage and report disengagement; coordinate with outreach team for disenrollment as needed.
• Perform care planning and therapeutic case management with a flexible, whatever-it-takes approach.
• Maintain data collection, reporting and charting records per policy; meet service unit requirements.
• Participate in supervision, meetings, travel, and trainings as required.
• Balance diverse client needs and adapt to change while maintaining quality service.
