Job summary

Humana Inc. is hiring a Case Manager/Community Resource Coordinator (CRC) to serve Wisconsin Family Care members.

This field position partners with an RN Wellness Coordinator to support plans of care and deliver social services in members’ homes and communities across Jefferson County, WI. The role involves face-to-face assessments, quarterly visits, monthly calls, and coordination of social, employment, housing, and non-health needs.

Qualifications

• Bachelor's degree in Human Services or related field (required)
• OR a Bachelor's in any field with 3 years related experience in Family Care populations

Responsibilities

• Learn what is important to the member and support them in achieving it.
• Partner with Health and Wellness Coordinator (RN) and collaborate on the member's plan.
• Perform face-to-face social-related assessments for enrollment and at least every 6 months.
• Coordinate, perform, and track quarterly face-to-face visits and monthly phone contacts.
• Participate in development and ongoing review of the member's plan of care.
• Lead in social, community integration, employment, housing, and non-health issues.
• Educate on prevention/wellness and mitigate risk when assessed.
• Document activity and complete paperwork.

Education

Bachelor's degree in Human Services or related field
