Job summary

You will complete assessments, develop care plans, conduct home visits as needed, monitor progress, and promote service integration across behavioral health, LTSS, and community resources. Travel 25-40% may be required.

Qualifications

• Experience in care management or health care settings.
• Knowledge of community resources and care coordination concepts.
• Licensing/certification requirements depend on state contract or regulation.

Responsibilities

• Completes assessments of members and determines eligibility for care coordination.
• Develops and implements care plans with member and care team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors progress and documents interventions and outcomes.
• Maintains ongoing member caseload with outreach and management.
• Promotes integration of services to enhance continuity of care.
• Facilitates ICT meetings and collaboration.
• Assesses barriers and provides assistance to address concerns.
• Collaborates with licensed care managers as needed.
• Travel up to 40% may be required.
