Qualifications

• 2+ years in health care, including care management or LTSS.
• Active RN license in state of practice.
• Bachelor's degree in a health care-related field may be required.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Perform comprehensive member assessments within regulated timelines, including home visits.
• Coordinate waiver enrollment/disenrollment and care plans with members and team.
• Monitor care plans and adjust interventions to achieve goals.
• Promote integrated services across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for approvals/denials.
