Qualifications

• At least 2 years experience in health care, preferably in care management.
• Clinical licensure/certification required only if state contract requires it.
• Valid driver's license, reliable transportation for travel.
• Knowledge of community resources.
• Ability to work proactively with attention to detail.
• Experience working with diverse populations and settings.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office and applicable software.

Responsibilities

• Completes member assessments within regulated timelines and identifies care coordination eligibility.
• Develops and implements care plans with member, caregivers, physicians and team.
• Conducts telephonic, in-person or home visits as required.
• Monitors care plan effectiveness, documents interventions and goals.
• Maintains ongoing member caseload and outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and clinical guidelines to support change.
• Assesses barriers to care and provides coordination and assistance.
• Collaborates with licensed care managers as needed.
• Travel 25-40% as required by state contracts.
