• Maintains ongoing member caseload and outreach.
• Promotes integration of services across care teams and LTSS.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing and Molina guidelines.
• Identifies barriers and provides care coordination assistance.
• Collaborates with licensed care managers as required.
• Travel 25-40% based on state requirements.
