Job summary

CVS Health is seeking a Care Management Coordinator for OhioRISE in the West Region of Ohio. This full-time, field-based telework role travels within Western Ohio up to 50% to member homes and locations. Typically 40 hours weekly, Mon-Fri 8am-5pm with flexibility for after-hours coverage.

The role emphasizes care coordination, collaboration with families and teams, and ensuring quality outcomes for children with behavioral health needs within Ohio Medicaid.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with PC use, keyboarding and MS Office Suite.
• 2+ years of experience in children's mental health, child welfare or related field.
• 2+ years in areas such as family systems, community resources, case management, or child development.
• 2+ years experience with Ohio delivery systems and local networks.

Responsibilities

• Evaluate referred members’ needs/eligibility and plan case resolution.
• Coordinate and monitor assigned care plan activities.
• Collaborate with case managers, supervisors and Medical Directors to improve care.
• Provide coaching and support to empower members to participate in healthcare decisions.
