Job summary

CVS Health is seeking a Care Management Coordinator for the OhioRISE program in central/southeast Ohio. This full-time field-based and telework role requires travel to member homes and sites up to 50% of the time.

Residency in central or southeastern Ohio is required, with preferred counties listed. Typical hours are Monday–Friday 8–5 with some after-hours as needed. Bachelor's degree in behavioral health or related field is required.

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector health/behavioral field.
• 2+ years in areas like family systems, case management, child development, or related.
• 2+ years experience with Ohio delivery systems and local networks.

Responsibilities

• Evaluate member needs/eligibility and recommend care plan actions.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with case managers, supervisors, Medical Directors and health programs.
• Provide coaching and empower members to participate with providers in decision-making.
