Qualifications

• Active IL RN license.
• 3–5 years clinical nursing experience.
• 2–3 years in case management, discharge planning, care coordination, or home health.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently in a remote environment.
• Proficient with MS Office and care management systems.

Responsibilities

• Serve as liaison among members, families, providers, employers, health plans, and stakeholders.
• Coordinate and manage care activities for members with complex health needs.
• Conduct comprehensive assessments of medical, functional, psychosocial, and environmental needs.
• Develop and monitor individualized care plans to improve health outcomes.
• Facilitate access to healthcare services, community resources, and home health.
• Engage with members via telephoner and in-person visits as needed.
• Collaborate with interdisciplinary care teams to ensure coordinated care.
• Communicate treatment plans and progress to providers and families.
• Educate members on disease management, benefits, and community resources.
• Document activities per regulatory and organizational requirements.
• Promote optimal health outcomes and quality of life.
