Qualifications

Responsibilities

• Perform comprehensive member assessments within required timelines, including home visits.
• Coordinate waiver enrollment and service plan development with the care team.
• Monitor care plans and adjust interventions to meet member goals.
• Promote integrated care across behavioral health, LTSS, and community resources.
• Assess medical necessity and authorize waiver services as appropriate.
• Lead ICT meetings for service approvals and coordinate cross‑functional collaboration.
• Travel 25–40% as required by state/contract.
