Job summary

Insight Global is seeking an experienced Care Manager or Home Health Nurse to support dual-eligible Medicare and Medicaid members with complex medical, behavioral, and social needs. The focus is conducting assessments to improve health outcomes and quality of life.

Ideal candidates have Massachusetts RN licensure or an advanced nursing/ social work degree, with 50%+ local travel and a valid driver's license. This community-based role requires strong care coordination and fieldwork across MA.

Qualifications

• Massachusetts license as RN or nursing degree or MSW/mental health counseling
• 3+ years RN/Behavioral Health Clinician experience, or 1 year RN/Behavioral Health + 2 years with complex populations
• Valid driver's license, reliable transportation, and auto insurance
• Must reside within commutable distance of Massachusetts and travel 50%+ for visits

Responsibilities

• Complete comprehensive and functional assessments and UCA Assessments (Uniform Core Assessment)
• UCA historically referred to as an MDS assessment

Job description

• Current Massachusetts license as one of the following:
• Registered Nurse (RN)
• Associate's Degree in Nursing or Master's Degree in Social Work/Mental Health Counseling
• Either:
• 3+ years of RN/Behavioral Health Clinician experience, or
• 1 year of RN/Behavioral Health experience plus 2 years working with complex populations
• Valid driver's license, reliable transportation, and auto insurance

Ideal Candidate:

Experienced Care Manager or Home Health Nurse with strong care coordination experience.

Preferred Experience

• LTSS and Dual-Eligible populations
• Population health or community health
• Behavioral health
• Case Management Certification (CCM) preferred

Day-to-Day Responsibilities

This community-based Care Manager supports dual-eligible Medicare and Medicaid members with complex medical, behavioral, and social needs. The primary focus is conducting assessments to improve health outcomes and quality of life.

Responsibilities include:

• Conducting in-home and community-based member visits
• Completing comprehensive assessments, functional assessments and/or UCA Assessments
• UCA = Uniform Core Assessment
• UCA used to be referred to as an MDS assessment
• Must reside within a commutable distance of Massachusetts
• 50%+ local travel required for member and provider visits
• Community/mobile-based position with significant fieldwork throughout assigned territory

Compensation

$50/hr to $55/hr.

Exact compensation may vary based on several factors, including skills, experience, and education.
