You will work across inpatient, outpatient, and community settings to ensure smooth transitions of care.

Qualifications

• Requires Florida licensure in one of the following: LCSW, LMHC, or LMFT.
• Minimum 2 years healthcare experience, preferably in care management or behavioral health.
• Valid driver's license and ability to travel locally (approximately 10%).
• Knowledge of community resources and transition of care.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes assessments of members per regulated timelines and determines eligibility for care coordination.
• Develops and implements care plans with member and care network to address needs and goals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors and documents progress toward goals and adjusts interventions accordingly.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health with LTSS and community resources.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing and Molina clinical guideposts.
• Assesses barriers to care and provides assistance to members.
• Collaborates with licensed care managers as needed.
• Approximately 10% local travel for member visits and coordination.
