Job summary

Humana Inc. seeks a Care Coordinator to support Illinois Medicaid HealthChoice and manage early-care coordination for new enrollees. You will perform HRAs, outreach, and connect members to services while navigating barriers to care. Expect up to 75% local travel to member sites and facilities.

The role emphasizes telephonic and in-person assessments, coordination with interdisciplinary teams, and guidance through the enrollee care journey to promote optimal well-being.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• 1+ years of health care and/or case management experience.
• Experience with Medicare & Medicaid recipients and/or long-term care or Home and Community based services.

Responsibilities

• Outreach to members and complete Health Risk Assessments (HRAs).
• Coordinate services and monitor the case management plan.
• Identify barriers to care and connect enrollees to needed services and resources.
• Assist enrollees with grievances and appeals.
• Ensure services are delivered in the least restrictive setting and aligned with enrollee goals.
