Help at Home is seeking a Field Coordinator to support in-home care coordination and client scheduling. The role involves daily client home visits, phone-based inquiries, and documentation of visits to ensure safe, quality care.

Ideal candidates have strong communication, good interpersonal skills, and basic computer proficiency. A valid vehicle and insurance are required for local travel, with roles based in the Chicago area to serve communities across the region.

Qualifications

• Strong communication skills, both oral and written.
• Excellent interpersonal skills for interacting with clients and families.
• Proficient computer skills for documentation and scheduling.
• HOST trained (new requirement) or willing to complete training.

Responsabilités

• Conduct daily visits to clients' homes.
• Answer incoming phone calls and redirect as needed.
• Greet and assist guests as required.
• Enter and submit documentation records of home visits.
• Assist the Branch Manager with administrative/clerical duties.
• Duties may be reassigned as needed by management.
