Arkansas Total Care is seeking a field-based care coordinator to support members across Phillips, Monroe, and Lee Counties, AR. You will engage members by phone or home visits, discuss care plans, and coordinate with providers and caregivers to adapt plans as needed.

You will document needs, communicate with care teams, and educate members on benefits and resources, while traveling about 30% of the time.

Formación

• Requires a High School diploma or GED.
• 1–2 years of related experience.
• Bachelor's degree in social science/health-related field OR HS diploma with relevant care coordination experience.

Responsabilidades

• Outreaches to members by phone or home visits to discuss care plans and next steps.
• Coordinate care activities with providers, partners, and family caregivers.
• Address member inquiries and assist with care plan documentation.
• Communicate with care managers and practitioners to ensure continuity of care.
• Support assessments and document member care needs as appropriate.
• Maintain member records per regulatory standards and distribute as needed.
• Follow policies and identify referrals to Care Manager or community resources.
• Provide education on benefits and resources; perform other duties as assigned.
