Job summary

The Field Care Coordinator position with UnitedHealthcare involves traveling within Winchester, Clarke County, and Frederick County, VA to assess needs and develop comprehensive, person-centered care plans in collaboration with an interdisciplinary team.

Qualifications

• Bachelor’s degree in Social Work or Human Services (or related field).
• 3+ years of care coordination, behavioral health experience, and/or work in a healthcare environment.
• 3+ years of experience with medical needs, elderly, individuals with disabilities, or communication barriers.
• 1+ years of experience with MS Office, including Word, Excel, and Outlook.
• Access to reliable transportation and a valid US driver’s license with ability to travel within assigned territory.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing chronic conditions, health promotion, and social determinants of health.
• Partner with internal care team, providers, and community resources to implement care plan.
• Provide education and coaching to support member self-management.
• Support proactive discharge planning and manage care transitions.
