Job summary

UnitedHealthcare is seeking a Field Care Coordinator to assess and plan care for enrollees, traveling to homes and facilities in Northern Virginia. You will collaborate with the interdisciplinary team to ensure comprehensive, cost-effective patient care and promote self-sufficiency.

This field-based role requires coordinating with families and providers, supporting transitions after ER visits or SNF admissions, and advocating for members’ choices with the health care team.

Qualifications

• Bachelor's degree in Social Work or Human Services (or related field).
• 3+ years of experience in care coordination or related healthcare setting.
• 3+ years of experience working with medically complex populations.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Willing and able to travel within assigned territory; valid US driver's license.
• Access to reliable transportation.

Responsibilities

• Engage members in comprehensive needs assessments including medical, behavioral, functional, cultural and socioeconomic factors.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, and safety.
• Collaborate with internal teams, providers, and community resources to execute the plan.
• Provide education and coaching to support self-management of care needs and lifestyle changes.
• Coordinate care transitions after ER visits or SNF admissions and assist with discharge planning.
• Advocate for members and families to represent their needs with the care team.
