Humana Inc. is seeking a Care Coach 1 to assess member needs, plan care, and connect them with services to improve wellbeing. The role involves meeting members in their homes or care settings and requires 75-90% local travel.

Requirements include a Bachelor’s degree with 2+ years in health services or RN/LPN Florida license, plus strong documentation and bilingual English/Spanish skills. Flexible schedule Mon–Fri with comprehensive benefits.

Qualifications

• Bachelor's degree with 2+ years of relevant health services experience.
• RN or LPN license in Florida with applicable experience.
• Case management experience and familiarity with Medicare/Medicaid.
• Strong computer skills and ability to document in systems.
• Bilingual in English/Spanish is advantageous.

Responsabilités

• Assess member needs to create a care plan.
• Ensure services in least restrictive settings to promote wellbeing.
• Plan and implement interventions for identified needs.
• Coordinate services and monitor the care plan against goals.
• Guide members toward appropriate care resources.
• Engage with payers, providers, and interdisciplinary teams as needed.
