Job summary

Quality Progressions is seeking a Supports Coordinator to engage individuals and families in planning that expands life opportunities. You will coordinate services, monitor delivery, and promote choice and control in line with agency values.

The role requires experience in supports coordination for people with intellectual/developmental disabilities, strong computer skills, and ability to work with diverse teams and families.

Qualifications

• Associate or Bachelor’s Degree in a relevant field of study, and a minimum of 12 credits in Social Science related coursework.
• Proven ability to pass background screens.
• Valid driver’s license and reliable car.
• Travel up to 50% of the time.

Responsibilities

• Completes assessments to identify each person’s needs and desires using formal and informal tools.
• Assists the person to develop their Individual Support Plan (ISP) and personal budget, updating annually.
• Supports the person to identify outcomes and ensures ISP addresses them.
• Identifies services to meet needs and coordinates referrals and service delivery.
• Monitors services to ensure delivery as planned and safe health status.
• Assists in identifying and sustaining the person’s support network.
