Job summary

Quality Progressions is seeking a Supports Coordinator to engage individuals and families in developing plans that address needs and expand life opportunities. You will coordinate services, monitor delivery, and promote individuality, choice and control for people with intellectual/developmental disabilities.

The role requires a strong background in supports coordination, proficiency with computers, and a customer-service focus.

Qualifications

• Associate or Bachelor’s Degree in a related field with at least 12 credits in Social Science coursework.
• Two years’ prior Human Services work experience required.
• Ability to pass background screens.
• A reliable car and driver's license; 50% travel required.

Responsibilities

• Completes assessments to identify each person’s needs and desires.
• Assists the person to develop their Individual Support Plan and personal budget, updating annually.
• Supports the person to identify outcomes and ensures ISP addresses them.
• Identifies and coordinates services, completes referrals, and resolves barriers.
• Monitors services to ensure delivery and safety, achieving identified outcomes.
• Assists in sustaining the person’s support network including family and friends.
• Develops community resources to preserve well-being at home and in the community.
• Attends training to stay current with best practices in services for people with intellectual/developmental disabilities.
• Trains families and consumers on rights and due process.
• Maintains professional relationships with families and consumers on caseload.
