Job summary

Pathways Forward in Harrisburg, PA is seeking a dedicated Supports Coordinator to join our Intellectual and Developmental Disability Program. This full-time role focuses on creating person-centered plans and coordinating services to help individuals live full, inclusive lives in their communities.

The ideal candidate has a Bachelor’s degree with relevant coursework or an associate degree with experience, holds required clearances, a valid driver’s license, and is willing to relocate to

Qualifications

• Bachelor’s degree with 12+ college credits in sociology, social work, psychology, gerontology, criminal justice, or related field; OR an associate degree with 12 credits and two years of relevant experience.
• Required clearances: PA Child Abuse, PA Criminal History, and FBI.
• Valid driver’s license and a reliable vehicle.
• Relocation to Harrisburg, PA, is required before starting.

Responsibilities

• Facilitate the creation of person-centered support plans with individuals, families, and support teams.
• Identify and refer community and system supports and services to meet individual needs.
• Coordinate and monitor the delivery of supports and services as outlined in support plans.
• Ensure the quality, type, duration, and frequency of services meet individual needs.
• Monitor health and wellbeing of participants through regular contact with individuals, families, and service providers.
• Identify unmet needs and document them for future planning and funding opportunities.
• Inform individuals of their rights and maintain comprehensive documentation through a statewide web-based system.
• Participate in ongoing training to enhance your skills and knowledge.

Education

Bachelor’s degree with 12+ college credits in sociology, social work, psychology, gerontology, criminal justice, or related social science; OR Associate degree with 12 college credits in the above fields and two years of relevant experience.

Schedule: Monday to Friday, Day Shift, No Weekends
