Alternative Family Services in San Mateo, CA is seeking a Support Counselor/Intensive Care Coordination to work in the field with the treatment team. You will focus on care coordination, help families build a strong treatment plan, and center the voice and needs of the child and family.

Qualifications

• Bachelor's or associate degree in a social science related field.
• Experience with emotionally disturbed children or behavioral problems preferred.
• Background clearances: fingerprint and government approvals.

Responsibilities

• Provide support services to the child and foster family per treatment approach.
• Deliver interventions such as coaching, mentoring, and skill building.
• Provide emergency response; some evenings and weekends may be required.
• Ensure safety and well-being of the foster child at all times.
• Transport children to appointments using a personal vehicle in safe order.
• Submit written documentation to CPS workers.
• Maintain HIPAA privacy practices and attendance policies.
• Collaborate with supervisor and treatment team on duties.
