The Harris Center for Mental Health and IDD is seeking a Care Coordinator to provide case management and care coordination for individuals in a Harris County housing program. You will work with a multidisciplinary team, including LPHA and LCDC, and may transport clients with mileage reimbursement.
Responsibilities include assessments, recovery planning, eligibility compliance, skills training, crisis intervention, and comprehensive documentation.

Under the general direction of the Clinical Team Lead, the CC provides case management and coordination of care services within the community for individuals participating in a housing program affiliated with the Coalition for the Homeless. The CC works as part of a multidisciplinary team that includes a Master Level Clinician or LPHA and LCDC, and also offers peer support services. The CC is expected to provide transportation to consumers, often using their personal vehicle and receiving mileage reimbursement. Through intensive crisis intervention, engagement, and care coordination, the primary focus is to maintain housing and mental health for participants. The Care Coordinator must hold a valid State of Texas driver’s license and required insurance coverage.
