Job summary

Activate Care is seeking a field-based Registered Nurse (RN) to perform in-home health assessments for dually eligible members across Massachusetts. Travel to members’ homes and community settings to complete timely assessments, document findings, and hand off to care management teams.

The role emphasizes assessment and documentation rather than ongoing care planning, with a focus on quality, safety, and compliance in a managed care environment.

Qualifications

• Active Massachusetts RN license in good standing.
• Associate Degree in Nursing required; BSN preferred.
• Valid driver’s license, reliable vehicle, and active auto insurance.
• 1+ year of home health clinical experience.
• 1+ year of assessment experience preferred (RAC-CT or similar).
• Experience with dual-eligible, Medicaid, or Medicare populations preferred.

Responsibilities

• Conduct in-person assessments in members' homes and community settings across MA.
• Document assessment findings in the health plan platform (GuidingCare) and send SOAP notes to Care Manager.
• Submit complete documentation within 48 hours of each assessment.
• Follow field-safety guidelines and report safety concerns per policy.
• Maintain HIPAA/NCQA compliance and complete mandatory trainings.
