Horizon Blue Cross Blue Shield of New Jersey is seeking a field-based registered nurse to assess members and develop individualized care plans. You will coordinate among patients, families, physicians, and hospitals while ensuring care is appropriate and cost-effective.

The ideal candidate holds an associates or bachelor’s degree in nursing, has at least two years of clinical experience, and maintains an active NJ RN license. This role requires daily local travel and strong communication skills.

Qualifications

• Associates or Bachelors degree in nursing (or higher).
• Minimum of two (2) years clinical experience.

Responsabilités

• Assess members' clinical need against guidelines to ensure medical appropriateness.
• Evaluate the necessity, appropriateness and efficiency of provided services.
• Develop, coordinate and implement individualized plan of care.
• Coordinate with patient, family, physician, hospital and external stakeholders regarding care.
• Monitor care and outcomes for appropriateness and effectiveness.
• Advocate for members across sites to optimize resources.
• Document accurately per standards and policies.
• Engage multidisciplinary teams to ensure continuity of care.
