Horizon Blue Cross Blue Shield of New Jersey is seeking a field-based Registered Nurse to perform assessments and develop member-centric care plans. You will work with patients, families, physicians, and hospitals to coordinate appropriate care along the continuum and promote high-quality outcomes.

The role requires an active NJ RN license, associates or bachelor’s degree in nursing, and a minimum of two years of clinical experience. Daily local travel is required in the New Jersey area.

Qualifications

• Requires an associates or bachelors degree (or higher) in nursing and/or accredited diploma nursing school.
• Requires a minimum of two (2) years clinical experience.

Responsabilités

• Assesses member's clinical need against guidelines to ensure services are medically appropriate.
• Evaluates necessity, appropriateness and efficiency of services provided.
• Develops, coordinates and assists in implementing an individualized plan of care.
• Coordinates with patient, family, physician, hospital and external customers regarding care from diagnosis to outcome.
• Monitors care activities and outcomes for appropriateness and effectiveness.
• Advocates for the member/family among multiple sites to coordinate resources.
• Encourages member participation and compliance in the program.
• Documents accurately based on standards and policies.
• Interacts with multidisciplinary teams to ensure continuity of care.
