Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a field-based nurse to assess members, develop individualized care plans, and coordinate care across providers to ensure appropriate and cost-effective outcomes.

The ideal candidate holds an active NJ RN license, can travel locally, and collaborates with patients, families and clinicians. Proficiency with MS Office and familiarity with care management is preferred; bilingual skills helpful.

Qualifications

• Active/unrestricted NJ RN license required.
• Associates or bachelor’s degree in nursing (or higher).
• Proficiency with Windows MS Office; Lotus Notes knowledge preferred.
• Experience in case/care management; knowledge of healthcare contracts helpful.

Responsibilities

• Assess members using established guidelines to develop a plan of care.
• Coordinate care with patients, families, physicians, hospitals and providers.
• Monitor and evaluate care outcomes across the continuum of care.
• Document accurately and participate in quality improvement activities.
• Travel daily to member sites; communicate with multidisciplinary teams.
