Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking an experienced Registered Nurse for a field-based role. You will assess members, coordinate plan of care, and collaborate with healthcare providers to ensure appropriate, high-quality, cost-effective care across the continuum.

The ideal candidate holds an associates or bachelor's degree in nursing, with an active NJ RN license and at least two years of clinical experience.

Qualifications

• Requires an associates or bachelors degree in nursing and/or accredited diploma nursing school.
• Active Unrestricted NJ RN License required.
• Minimum of two years clinical experience.

Responsibilities

• Assesses member's clinical need against guidelines to ensure medically appropriate services.
• Evaluates necessity, appropriateness and efficiency of services.
• Develops and coordinates member-centered care plans.
• Coordinates with patient, family, physician, hospital and other external customers.
• Monitors care across the continuum to ensure quality and outcomes.
• Advocates for members to optimize resource utilization.
• Documents accurately based on policies and standards.
• Collaborates with multidisciplinary teams; communicates for continuity of care.
