Horizon Blue Cross Blue Shield of New Jersey is seeking an RN for a field-based role that assesses members against guidelines and develops a member-centric care plan. You will coordinate with families, physicians, and hospitals to ensure medically appropriate, cost-effective care across the continuum.

Ideal candidates have an associates or bachelor's in nursing and at least two years of clinical experience, plus an active NJ RN license.

Formación

• Associates or Bachelor's degree in nursing or higher.
• Active NJ RN license required.
• Minimum of two years clinical experience.

Responsabilidades

• Assess members' clinical needs against guidelines to ensure medical appropriateness.
• Evaluate necessity, appropriateness and efficiency of services.
• Develop, coordinate and assist in implementing individualized plans of care.
• Coordinate with patient, family, physician, hospital and external stakeholders.
• Monitor care across the continuum and outcomes for appropriateness/effectiveness.
• Document accurately according to standards and policies.
• Participate in multidisciplinary teams and quality improvement programs.
