Horizon Blue Cross Blue Shield of New Jersey is seeking a field-based nurse to assess members and develop member-centric care plans. You will collaborate with patients, families, physicians and hospitals to ensure appropriate, high-quality care across the continuum.

Requires an associate or bachelor's degree in nursing and an active NJ RN license with at least two years of clinical experience. Travel is daily within the service area.

Qualifications

• Associates or bachelor’s degree in nursing or higher.
• Active Unrestricted NJ RN License required.
• Minimum of two years clinical nursing experience.

Responsabilités

• Assess members' clinical needs against guidelines to ensure appropriate care.
• Evaluate necessity, appropriateness and efficiency of provided services.
• Develop and coordinate member-centered plans of care.
• Coordinate with patient, family, physician, hospital and external stakeholders.
• Document care accurately per standards and policies.
• Collaborate with multidisciplinary teams to ensure continuity of care.
