Job summary

CVS Health seeks a compassionate Registered Nurse Case Manager to support members in Northern Virginia, including Fairfax, Sterling, Reston, and Ashburn. This field-based role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.

You will assess needs, develop care plans, coordinate across providers, and advocate for members with complex conditions, traveling up to 75% and leveraging multiple documentation platforms.

Qualifications

• Active, unrestricted RN license in Virginia.
• Residence in Fairfax, Sterling, Reston, Ashburn and surrounding Northern Virginia areas.
• Must be willing and able to travel up to 75% with mileage reimbursed.
• 3+ years of clinical nursing experience (medical-surgical, acute care, or behavioral health).
• Strong communication, organization, and critical-thinking skills.
• Comfort working with technology and multiple documentation platforms.

Responsibilities

• Conduct comprehensive assessments (telephonic and face-to-face) of members' medical, behavioral, and social needs.
• Develop and implement personalized care plans to promote short- and long-term wellness.
• Coordinate care across providers and services for a seamless member experience.
• Identify risks and gaps in care using clinical data insights.
• Support members with complex conditions through holistic, whole-person care.
• Facilitate referrals to clinical and community resources.
• Participate in case conferences with multidisciplinary teams.
• Review claims history and benefit eligibility to inform care planning.
• Engage members with strong interviewing to encourage participation.
• Ensure case management activities comply with regulatory guidelines.
