Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager to deliver high-acuity home health care across Texas. You will own patient assessments, document visits, and coordinate care with physicians, families, and the care team to ensure a strong plan of care.

This field-based role offers autonomy with a productivity-based pay model (59.10 per point) and an expected 30 points per week, along with a full-time Thu–Mon schedule.

Qualifications

• Requires Texas RN license and 3+ years of nursing experience.
• Comfort with wound care and infusions.
• CPR certification (BLS) required.
• Texas driver’s license and ability to work field-based.
• Strong time management and organizational skills.
• Clear, professional communication with patients and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care focusing on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Coordinate care and follow-ups to keep patients progressing toward goals.
• Coordinate with internal teams to ensure safe, reliable care delivery.
• Supervise LVNs and HHAs when applicable.
• Participate in case conferences and in-services.
