Job summary

Adaptive Home Health in Texas is seeking a Registered Nurse (RN) Case Manager to deliver high-acuity skilled home health care in the field. You will own patient assessments, document visits, and coordinate care with physicians, families, and care teams.

The role offers a full-time schedule (Monday–Friday with some weekend coverage) and a productivity-based compensation model at 58.80 per point with 30 points per week, plus mileage reimbursement.

Qualifications

• Current Texas RN license
• 3+ years of clinical nursing experience
• Comfort with wound care and infusions
• CPR (BLS) certification
• Current Texas driver's license and field-based work
• Strong time management and organization in a high-autonomy environment
• Clear, professional communication with patients, families, and providers

Responsibilities

• Own patient assessments and skilled visit documentation in the field.
• Deliver high-acuity home health care with wound care and infusion therapy.
• Coordinate care with physicians, care team, patients and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management including follow-ups and barrier resolution.
• Coordinate with internal teams to ensure reliable and safe care.
• Provide supervision to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to improve quality and outcomes.
