Job summary

Adaptive Home Health is seeking an RN Case Manager to deliver high-acuity home healthcare in Texas. You will assess patients, document visits including OASIS, and coordinate care with physicians and the care team. The role emphasizes wound care, infusion therapy, and advocacy for patients and families.

You will be part of a mission-driven team with supportive operations, and you’ll work in a field-based capacity with flexible scheduling and a focus on outcomes and safety.

Qualifications

• Active Texas RN license and 3+ years nursing experience.
• Experience in wound care and infusions.
• CPR (BLS) certification.
• Field-based work with a valid driver’s license.
• Strong time-management and independent work style.
• Excellent communication with patients and families.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with focus on wound care and infusion therapy.
• Maintain a clear plan of care through ongoing communication with physicians and care team.
• Advocate for patients and ensure continuity of care across disciplines.
• Coordinate care and follow-ups to progress toward goals.
• Coordinate services (PT/OT/ST/MSW/HHA) and manage orders.
• Administer medications and provide patient teaching.
• Supervise LVNs and HHAs when applicable.
• Participate in case conferences and in-services on quality and safety.
