Job summary

CVS Health is seeking a compassionate Registered Nurse Case Manager to support members in the Danville & Martinsville, Virginia area. This field-based role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.

You will conduct assessments, develop care plans, coordinate across providers, and review claims history to promote better outcomes while adhering to regulatory standards. Anticipated 40 hours per week.

Qualifications

• Active RN license in Virginia required.
• Minimum 3 years clinical experience (hospital, home health, ambulatory care, or similar).
• Residence in Danville & Martinsville area, VA; willing to travel locally.
• Comfort with in‑home visits and local travel.

Responsibilities

• Coordinate care across providers and services to ensure a seamless patient experience.
• Conduct comprehensive assessments (telephonic and in person) of medical, behavioral, and social needs.
• Develop and implement personalized care plans promoting short‑ and long‑term wellness.
• Review claims history and benefit eligibility to inform care planning.
• Engage members to encourage active participation in their care and ensure compliance with guidelines.
