Job summary

Humana is seeking an RN Field Care Manager in Virginia to perform telephonic and face-to-face assessments, develop care plans, and connect members with services to promote well-being. You will assess medical needs, monitor progress, and work independently within established guidelines to support the aged, blind, and disabled population.

The role requires active Virginia RN licensure, 2+ years in health care/case management, and travel within the Central Region.

Qualifications

• Active RN license in Virginia required.
• 2+ years in health care and/or case management required.
• 1+ year working with Cardinal Care Priority Population criteria preferred.
• Certification in Case Management (CCM) preferred.

Responsibilities

• Perform telephonic and face-to-face assessments and evaluations of member needs.
• Develop individualized care plans for members.
• Connect members with quality services to promote well-being.
• Analyze medical complexity and identify barriers to effective care.
• Monitor member care through comprehensive assessments and evaluations.
• Work independently with minimal direction.
• Follow established guidelines and procedures.
• Engage with members in their homes or locations of choice.
• Utilize evidence-based strategies to manage members' health.
• Collaborate with colleagues covering the aged, blind, and disabled member population.
