Job summary

Activate Care in Massachusetts seeks a part-time Registered Nurse to travel to members’ homes for in-person health assessments for a managed care health plan. This field-based role focuses on timely assessment and precise documentation, supporting the care management team.

Requirements include MA RN licensure, driving, and the ability to work independently, with flexible hours based on member availability. Vaccination and HIPAA compliance are expected.

Qualifications

• Active RN license in Massachusetts.
• Home health experience preferred.
• Able to document assessments accurately in electronic systems.

Responsibilities

• Engage with members in homes and community settings to complete assessments with cultural and linguistic awareness.
• Perform required assessments (e.g., UCA, FA, Comprehensive Assessments) and evaluate physical, cognitive, and functional status.
• Document assessments in GuidingCare per workflows and submit SOAP notes to Care Manager within 48 hours.
