Job summary

RCM Healthcare Services seeks a Registered Nurse for the Coordinator of Care role in Home Care (CHHA). This field-based position covers Manhattan, Brooklyn, and Queens, requiring a valid driver's license and reliable transportation.

You will coordinate, implement, and continuously evaluate patient care plans, collaborating with families and caregivers to tailor each plan. The ideal candidate holds an Associate’s Degree in Nursing (Bachelor’s preferred) with at least 1 year of acute care

Qualifications

• Associate’s Degree in Nursing is required; Bachelor’s preferred.
• Minimum of 1 year of acute medical-surgical nursing experience.
• Bilingual fluency in Spanish, Cantonese, or Mandarin is a plus.

Responsibilities

• Develop, implement, and evaluate individualized care plans for home care patients.
• Review patient insurance information and coordinate care activities.
• Share critical findings with patients’ families and interdisciplinary caregivers.
• Manage time efficiently to ensure optimal care delivery.
