Job summary

Affirmed Home Care, Inc. in Manhattan, NY is hiring a Full-Time Registered Nurse Care Manager to oversee a field-based caseload across Manhattan.

The role requires a NYS RN license, BSN (preferred), and 2+ years in clinical home care with advanced care skills including ventilator and tracheostomy care. You will assess clients, develop care plans, obtain physician orders, supervise field staff, and collaborate with families and physicians to ensure exceptional, client-centered care in the home

Qualifications

• Current New York State RN license in good standing.
• BSN preferred to support advanced care planning and supervision.
• Valid BLS certification required.
• Minimum 2 years of recent clinical home care experience.
• Hands-on clinical experience with advanced care (ventilator, tracheostomy, G-tube, IV infusions, wound care).

Responsibilities

• Conduct comprehensive client assessments and ensure highest quality of care.
• Develop, implement, and update individualized care plans.
• Obtain physician orders for home care services per NY regulations.
• Monitor clients and report changes to physicians and care team.
• Conduct supervisory visits and support field staff.
• Collaborate with clients, families, caregivers, physicians, and care team.
• Ensure care plans are followed and client needs addressed.
• Support client safety, independence, and quality of life at home.
• Maintain accurate clinical documentation.
• Travel throughout Manhattan to conduct field visits and support clients and caregivers.
