Job summary

Position Summary

One of our esteemed clients, a leading Certified Home Health Agency (CHHA), is seeking dedicated Registered Nurses to join their team as Coordinators of Care. These field-based roles cover Manhattan, Queens, and Brooklyn, with a car and valid driver’s license required for Queens and Brooklyn regions.

In this role, you will be the cornerstone of your patients’ home care journey, taking primary responsibility for coordinating, implementing, and continuously evaluating their care needs. You’ll collaborate with families and caregivers, ensuring that each care plan is effectively executed and tailored to the individual.
