Job summary

Path Ihc seeks an Outpatient Plus Certified Peer Support Specialist (CPSS) to deliver intensive, community-based wrap-around support for individuals with complex behavioral health needs. The CPSS leverages lived recovery experience to help clients stay stable, avoid hospitalization, and coordinate community reintegration in real-world settings.

Qualifications

• Lived experience as a person in sustained recovery from mental health condition or substance use disorder for at least 2 consecutive years.
• Valid driver’s license with reliable transportation.
• Proven ability to track, log, and manage billable units across field visits and time-tracking requirements.

Responsibilities

• Deliver flexible, individualized support in the client’s environment (home, community, social service offices).
• Collaborate within a multidisciplinary team to execute client care plans and coordinate care.
• Identify early warning signs of psychiatric or substance-related issues and support safety planning.
• Model self-advocacy and assist in reintegration, including housing and connection to recovery communities.
• Document face-to-face encounters and phone support for billing within required timelines.
• Complete timely, audit-ready EHR progress notes following state Medicaid definitions.
