Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and ability to work independently.
• Ability to travel daily across assigned territory.
• Strong communication and documentation skills.
• Experience using Homecare Homebase (HCHB) or similar EMR.

Responsibilities

• Own caseload as case manager and manage therapy plan and outcomes.
• Perform comprehensive in-home PT assessments and visits.
• Develop plan of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set goals, track progress, and adjust treatment.
• Coordinate with care team and supervise PTA where appropriate.
• Coach patients and caregivers with home programs.
• Document promptly and thoroughly in HCHB.
