Job summary

Liberty Healthcare Corporation, in partnership with the Pennsylvania Adult Protective Services program, seeks Protective Services Caseworkers to serve adults living with disabilities in Northwest Pennsylvania, including Warren and nearby counties. The role is field-based with autonomy and a flexible schedule.

You will travel, conduct residential investigations, interview individuals, document findings, develop service plans, coordinate services, and manage cases to closure.

Qualifications

• Bachelor’s degree in social work, criminal justice or human services preferred.
• Experience in service coordination, supports coordination, program specialist, or case manager roles encouraged.
• Ability to travel across regions and handle emotionally challenging situations.
• Own insured vehicle required for regional travel.

Responsibilities

• Travel throughout Northwest Pennsylvania focusing on Warren and surrounding counties.
• Conduct investigations in residences.
• Interview individuals and gather information.
• Identify, mitigate, and eliminate areas of risk to the individual.
• Document findings and write reports.
