Job summary

Elevance Health seeks an Advanced Practice Clinician - Field-Based Care to collaborate with company physicians and other providers to develop complex care plans in the patient’s home across a designated territory. Provides clinical and non-clinical support to patients in palliative care settings.

The role involves building and implementing plans of care, performing history and physicals, coordinating with the inter-disciplinary team, educating patients and families on medications and adherence,

Qualifications

• Requires an MS in Nursing.
• Requires current NP or PA license in applicable state(s). DEA registration or eligibility required.
• CPR certification required.
• Travel may be required.
• Bilingual in English/Spanish preferred.

Responsibilities

• Provides services in home settings to care for patients.
• Develops and implements clinical plans of care for adults with chronic/complex conditions.
• Gathers history, physical examination and diagnostics to develop treatment plans.
• Coordinates with interdisciplinary team and family to meet patient needs.
• Prescribes medications as permitted by state authority.
• Maintains patient records per state regulations and policy.
