Job summary

Elevance Health is seeking an Advanced Practice Provider - Field Based Care to deliver in-home care across the Xenia, Cedarville, and Yellow Springs areas, collaborating with physicians and a broader care team to develop and execute complex care plans aligned with patient goals.

You will travel to client sites, provide clinical and non-clinical support, educate patients and families, maintain records, and participate in ongoing state-required training.

Qualifications

• Requires an MS in Nursing.
• Requires current unrestricted NP or PA license in applicable states; DEA registration or eligibility required.
• CPR certification required.

Responsibilities

• Provides home-based care to patients.
• Develops and implements clinical plans of care for adults with chronic/complex conditions.
• Gathers history, exams, and diagnostics to tailor treatment plans.
• Coordinates with interdisciplinary team to meet patient and family needs.
• Educates patients and families about medications, adherence, and crisis prevention.
• Maintains patient records per state regulations and company policy; participates in continuing education; may prescribe medications as allowed.
