Job summary

Jobtailor seeks a Nurse Practitioner or Physician Assistant to provide high-quality palliative home care across the designated territory, collaborating with physicians, families and an inter-disciplinary team to develop and implement care plans.

Required: MS in Nursing, active NP/PA license in the state, DEA registration or eligibility, CPR, and TB clearance. Preferred: English/Spanish bilingual, 2 years in home-based or hospice care, and EMR experience.

Qualifications

• MS in Nursing required.
• Current NP/PA license in applicable state(s).
• DEA registration or eligibility.
• CPR certification required.
• 2 years of home-based, complex care, hospice or palliative care preferred.
• Bedside hospital/oncology/ER/ICU experience preferred.
• EMR experience preferred.
• English/Spanish bilingual preferred.
• TB test clearance required.

Responsibilities

• Collaborate with physicians and families to develop complex care plans.
• Provide clinical and non-clinical support to palliative care patients in the designated territory.
• Develop and implement clinical plans of care for adults with chronic/complex conditions.
• Gather history, exam, and diagnostics to create treatment plans.
• Meet physical and psychosocial needs with the interdisciplinary team.
• Educate patients and families on medications, side effects, adherence, crisis prevention.
• Coordinate care with other clinicians and providers.
• Maintain patient records per state laws and company standards.
• Participate in continuing education.
• Prescribe medications as permitted by state authority.
