Job summary

Elevance Health is seeking an Advanced Practice Provider for field-based care in the Bridgeport region. This role collaborates with physicians to develop complex care plans and delivers clinical support to patients in palliative care within a designated territory.

Responsibilities include creating treatment plans, educating patients and families, coordinating care with the inter-disciplinary team, and maintaining medical records in compliance with state regulations. Travel is required.

Qualifications

• Requires MS in Nursing.
• Requires current unrestricted NP or PA license in applicable state(s). DEA registration or eligibility required.
• CPR certification required.
• 2 years of home-based services, complex care cases, and hospice/palliative care preferred.
• Bedside hospital/ER/ICU experience highly preferred.
• Travel may be required; EMR experience preferred.

Responsibilities

• Collaborates with physicians and other providers to develop complex care plans.
• Provides clinical and non-clinical support to patients in palliative care within a designated territory.
• Develops and implements plans of care for adults with chronic/complex conditions.
• Gathers history, performs exams as needed, and creates treatment plans aligned with goals of care.
• Maintains patient records and coordinates with interdisciplinary team to ensure timely, compliant care.
